Thank you for participating in Phase 2 of the Innovation Initiative (i2) Challenge Grant! We appreciate the time, thought, and energy you’ve invested in refining your ideas and developing your early-stage models through this process.

As we close out Phase 2, each participating organization is required to submit:

1. Business Plan (3-5 pages)
2. Pitch Deck for a 5-minute in-person pitch scheduled for June 24, 2025 at the offices of Alliance Healthcare Foundation.

Both deliverables are due by Friday, June 20 at 5:00 PM PT and will serve as a demonstration of your proposed solution and business model.

Up to 5 organizations will be selected to advance to the final phase of i2 based on the following evaluation criteria:

• Innovative
• Community-Informed
• Potential Impact
• Feasibility
• Evidence-informed
• Leadership Team
• Potential for Scale and Sustainability
• Additionality through i2 Award
• Market understanding, market readiness, and market acceptance
